  NEPEAN HACC ACCESS PROJECT

Nepean HACC Access ProjectThe project responds to everyone's fundamental right to be able to access  services and participate in support and help provided to the members of the Australian community. 

The project is auspiced by TRI Community Exchange and receives re-current funding from the Department of Aging, Disability  & Home Care (DADHC) under the Home & Community Care (HACC) program.  The target group is NESB/CaLD frail older people, people with disabilities, their families and carers living in 3 local government areas (Penrith, Blue Mountains & Hawkesbury) that compose the Nepean Area.

The two main objectives of the project are:

  • To increase awareness of and access to  HACC funded services in the Nepean Area for frail older people and people with disabilities from CaLD backgrounds, their families and carers, and
  • To assist, support & resource HACC funded services in providing CaLD appropriate care/services to meet the needs of their clients.

To achieve our objectives we provide Training, Resources, Information & Support etc to HACC Service Providers and CALD Communities.

  • HACC Consumer & Carer Handbook

The Nepean HACC Access Project with the support of a working party of local HACC service providers/organizations developed "A Consumer & Carer Handbook" The booklet provides basic information about the Home & Community Care (HACC) program, eligibility, what services are available and how to access them.  

The booklet is useful for all service providers, health & community workers to leave with their clients during assessments, as it will provide the consumer with information to make informed choices.

The booklet has translated versions in Arabic, Spanish, Italian, Chinese and Greek.  Translations in other languages will be completed as our budget allows.

This handbook is free to all HACC services in the Blue Mountains, Penrith & Hawkesbury LGAs. A CD ROM version (includes translations) is available to non-HACC services for $5.50 (inc. GST).

  • "Profile of Older People of Culturally and Linguistically Diverse Backgrounds (CALD) Who Live in Penrith, Hawkesbury & Blue Mountains Local Government Areas" 

The Profile has been compiled to assist Home & Community Care (HACC) services & HACC-like services in the Nepean area to better understand the community in which they work and to provide them with a tool for planning and targeting their services to people from CALD backgrounds.

This profile can help HACC and HACC like services in a number of ways:

  • Services wanting to translate service promotion brochures into other languages can identity which are the most commonly spoken languages by older people in the Nepean area
  • Services wanting to embark on a comprehensive process for making their service accessible can use the profile as a first step for identifying the diversity of backgrounds and languages spoken in the Nepean area
  • This profile is an essential tool for services responding to the HACC National Standards

Demographic information is only a beginning point and can only ever paint a broad picture of the community. Demographic and other statistical information needs to be complemented with local knowledge and community consultation with ethnic communities. 

We hope that this profile will also be used by non-HACC organisations which work with or are interested in the needs of older people and people of culturally and linguistically diverse backgrounds.

This resouce is available free to all HACC services in the Blue Mountains, Penrith & Hawkesbury LGAs. It is also available for sale, on print ($22.00 inc. GST, limit 1 per organisation) and CD ROM ($5.50 inc. GST).
